All players must submit a completed 2018 player registration packet including financial, health and code of conduct forms before registration can be processed. All applications must be accepted by a Burbank United SC official administrator and/or team manager.
Attach / Copy of birth certificate (new players): Your team manager will need a certified copy of the birth certificate for our club registrar. Under NO circumstances will a hospital birth records be accepted. According to CYSA regulations this constitutes an ineligible player violation, and all games this player participates in are subject to forfeit. Complete & Print 2019 Registration Packet Here
You may pay your club fees either by check or with a credit card “this is seperate from team fees. (Credit card payments may be made from the “PAY YOUR BALANCE” button on the bottom of this page.) If you pay with a check, attach the check with registration fees to your registration packet. All checks for club payment(s), must be presented at the day of your player’s registration. All checks must be payable to Burbank United Soccer Club. No player card will be issued without proof of payment.
Per club policy, the complete uniform (including BOTH maroon AND sky blue jerseys, black shorts & socks) and Adidas Climate practice tee are required purchases, with the Club designated warm up & Adidas black backpack being optional. The team manager will manage the numbers for all players. Please make sure you purchase the correct size and for the correct gender. There are no returns allowed.
